The Frog Tape rebate form is here. They require that you buy 1 of their products and then send in the form. This may be an expensive product, you'll have to decide if it's worth it!

Both the rebates will accept a photocopy of the receipt with your purchase on it. They also both require a UPC from the movie. Because Disney movies always have 2 UPC's on their DVDs, you can send in both rebates!
